The Toyota Production System: What Does It Mean, And What Does It Mean For Health Care?

According to the Puget Sound Business Journal, Dr. Gary Kaplan, CEO of Virginia Mason Medical Center in Seattle Washington, was named one of the “50 Most Influential Physician and Executive Leaders” for the eleventh year.

He was most recently contracted to help with cost and error reductions at five major hospitals in England’s National Healthcare System.

Buffalo News Report: ‘Lean Six Sigma’ comes to the VA; Collins Cheers

Veterans Affairs Secretary Robert A. McDonald is relying in part on Lean Six Sigma to reorganize the embattled VA healthcare system. As reported in The Buffalo News article ‘Lean Six Sigma’ comes to the VA; Collins Cheers, this gives Chris Collins reason to celebrate. Collins, a current Republican Congressman from Clarence, NY, championed Lean Six Sigma as a method to streamline county government when he served as Erie County Executive beginning in 2007. Although he lost a re-election bid in 2011, McDonald’s plans to use the system provides some vindication for Collins, who credits the method for some of his fiscal and policy successes in Erie.‘Lean Six Sigma’ comes to the VA; Collins cheers

McDonald, in a speech on November 6th, explained how the system, combined with another management tool called Human Centered Design, can help the VA:

Great customer service companies use Human Centered Design to understand what customers want and need, and then design customer experiences to meet those needs. Lean Six Sigma makes these processes effective, efficient and repeatable.

For those unfamiliar with the process, Lean Six Sigma focuses on eliminating waste in business operations while at the same time reducing variability in production to maximize efficiency. McDonald believes these methods can help even in a human-services oriented setting like the VA. Wait times for appointments have already been significantly reduced for veterans, and the agency is in the process of decreasing the time it takes to process claims.

McDonald, a retired chairman of Proctor & Gamble, has responded to criticism that the tool is meant for manufacturing corporations and not a large health care network by saying: “People want to have a purpose. That’s belief No. 1.” In other words, using Lean Six Sigma will help bring focus back to the clients and involve both patients and providers in the process of improvement.

Meanwhile, Congressman Collins is hopeful that effective change will finally come to the agency. Some of the problems at the Buffalo VA included reuse of contaminated needles and poor management of patient records. “I have great hopes that this will make for a profound, positive change,” he said.

“The methodology of quality combined with the methodology of TeamSTEPPS is absolutely attainable.  The two very clearly support each other,” said workshop participant Lori Scanlan-Hanson, a registered nurse and quality specialist at the Mayo Clinic.  She was excited to see Lean Six Sigma being taught at the TeamSTEPPS conference, explaining that the two systems share similar philosophies and can be combined to create an environment that fosters trust and safety.  “I think the way to go is to just integrate the two,” she said.

Indiana Hospital Association Lauds Patient Safety Efforts – Lean Six Sigma in Action

By employing the [Lean Six Sigma] techniques learned through Coalition for Care over the past three years, Franciscan Healthcare-Munster has reduced patient harm by 40 percent and readmissions by 20 percent

We’ve recently discussed how Lean Six Sigma can make a difference in the healthcare industry by reducing wait times in the emergency department and improving patient flow, but a group of hospitals in Indiana is proving that Lean Six Sigma has even broader benefits.  The Indiana Hospital Association recently praised patient safety efforts at Franciscan Healthcare-Munster, one of 116 Indiana hospitals that has successfully employed Lean Six Sigma training to improve patient safety.

This state-wide patient safety initiative, known as Coalition for Care, has been a collaborative effort between the Indiana Hospital Association (IHA), the American Hospital Association’s Health Research and Educational Trust (HRET), and the 116 participating hospitals.  IHA and HRET offered education sessions and training for representatives from each hospital, focusing on topics such as readmission, patient falls, and patient and family engagement.

Lean Six Sigma training was an integral part of this educational initiative.  During the three years of collaboration, 176 Lean Six Sigma black belts were trained.  These trainees learned how to reduce inefficiencies and improve processes, enabling them to optimize patient care.

By employing the techniques learned through Coalition for Care over the past three years, Franciscan Healthcare-Munster has reduced patient harm by 40 percent and readmissions by 20 percent.  Reflecting on the success at Franciscan Healthcare-Munster, Indiana Hospital Association president Doug Leonard noted:

This is proof that efforts to identify harms and reduce readmissions are working. Our hospitals should be commended for their continued commitment to improving patient care. Indiana hospitals have had many accomplishments in the past three years, and will continue this work in 2015 to improve the overall health and well-being of our state.

In fact, an independent comparative study of 13 Lean projects, staff at all levels reported higher employee satisfaction at every institution that used the approach. The study reported that better front-line staff involvement in problem-solving and employee collaboration were cited as reasons for the results.

Sadly, a recent analysis by the  Centers for Medicare and Medicaid Services (CMS) estimated that 13.5% of Medicare beneficiaries experienced an adverse event during a hospital stay. This lack of progress demands that healthcare, as an industry, examine new ways to improve their operations. Lean Six Sigma’s long history of successfully reducing errors and costs in other industries demands the attention of the healthcare sector. Reports of successes such as those highlighted above indicate that the approach can provide benefits to all healthcare stakeholders. I applaud these efforts and urge other healthcare organizations to investigate these success stories as possible role models for themselves.

Caution! Your Hospital Stay Could Be Fatal

Anyone who has studied Six Sigma knows that these processes are performing far below Six Sigma levels. Why are we willing to tolerate such abysmal quality from such an important supplier?

The Sunday paper’s Parade section on February 8, 2009 contained an interesting article entitled “Don’t Let a Hospital Make You Sick.” The article contained a number of facts that I knew, but that are not widely known by the general public. Facts such as 1300 times per year operating on the wrong patient or removing the wrong organ or limb. Or 98,000 deaths per year due to medical mistakes. Or surgical instruments being left inside patient at a rate of once per 5000 patients.

Anyone who has studied Six Sigma knows that these processes are performing far below Six Sigma levels. Why are we willing to tolerate such abysmal quality from such an important supplier? I’ve worked in healthcare enough to know that there’s no reason that our healthcare can’t be delivered with at least as high a quality level as a beer can.
